Tamjidi Skin Institute (“TSI”) respects the privacy of users of our web site. This Privacy Statement explains our online information practices regarding the collection of personally identifiable information. Personally identifiable information is information, such as your name, physical address, e-mail address, telephone number and medical information. If you provide us with personally identifiable information, we will treat it in accordance with this Policy. There are three areas of the NSPC web site that require users to provide personally identifiable information:
1) Online Prescription Refill Request
Necessary Data to Complete Transaction: Patient Name, Telephone Number(s), Date of Birth, E-mail Address, Pharmacy Information, Drug Information (e.g., name, strength, frequency)
Necessary Data to Complete Transaction: Patient Name, Treatment Information (e.g., type of procedure, area treated, physician name), E-mail Address, Telephone Number(s), Testimonial Statement
Necessary Data to Complete Transaction: Patient Name, E-mail Address, Telephone Number, Comments/Feedback
You may choose at any time not to provide personally identifiable information to us, although this may restrict our ability to provide requested services or information. TSI uses the personally identifiable information collected from you for the purposes for which it was submitted. We may also use your information to provide you with better service and communicate back to you. With your specific permission or written authorization to the extent the testimonial includes protected health information; we may share your testimonials with other future patients on our web site.
Web Site Technology
Non-Personally Identifiable Information. TSI’s web server logs information that does not, by itself, identify a specific individual. We collect certain information about the use of our web site by our visitors. This information is then compiled and analyzed in a way that does not identify our visitors personally. The information may include the Uniform Resource Locator (URL), indicating from where a visitor to our web site has come, which URLs have been visited on our web site and in what order, the date and time of your visit, what browser is being used and what Internet Protocol (IP) address a visitor used to get to our web site. We use this information for web site and system administration to improve our service.
Linked Web Sites. We may provide links to third party web sites. You acknowledge that such links are provided for your convenience only and do not constitute an endorsement by TSI of such third party web sites. We do not control those web sites and assume no responsibility for the privacy practices of those web sites. We encourage you to review the privacy policies posted on those, and all third party web sites.
Protecting Your Information
We provide reasonable protections for the personally identifiable information you share with us. Unless we have your permission or are required or permitted by law, we will only share the personally identifiable information you provide with authorized TSI staff and third party service providers that perform services on our behalf, all of whom are required to keep your information confidential. TSI does not sell, loan, trade or lease any personally identifiable information collected on our web site to unrelated third parties for their own purposes.
To prevent unauthorized access, maintain data accuracy, and ensure proper use of your personally identifiable information, TSI employs appropriate physical, electronic and managerial procedures to safeguard and secure the information we collect online, consistent with industry practices. You should keep in mind that when providing personally identifiable information via e-mail to TSI, e-mail messages can be viewed by other Internet users while in transit. Electronic transmissions via the Internet are not necessarily safe from interception. TSI cannot guarantee the security or confidentiality of such transmissions.
For the services requiring personally identifiable information, TSI uses a technology called Secure Socket Layer (SSL). If your browser is capable of SSL (and most are), your personally identifiable information will be protected during transport across the internet.
We do not knowingly collect personally identifiable information from children under the age of 18. If we learn that we have personally identifiable information on a child under the age of 18 we will delete that information.
